Clinical exercise support for case managers

We understand the challenges of navigating the next stage in your client’s rehabilitation journey. Our expert team collaborates closely with physiotherapists and other healthcare professionals to create clinical exercise programmes that address your client’s unique needs and goals.

Focusing on achieving independence

Whether supporting home visits, transitioning clients into the community or gym, or introducing pool-based exercise, we tailor each plan to fit seamlessly with their ongoing rehabilitation.

Our approach focuses on exercise adherence, ongoing rehabilitation, and the development of independence.

How we support case managers and their clients

We work as part of a multidisciplinary team to ensure our clinical exercise plans are aligned with existing treatment plans and contribute directly to clients’ recovery goals. Our services cater to a wide range of conditions, including:

  • Post-injury rehabilitation
  • Chronic condition management
  • Long-term strategies for functional independence
  • Neurological conditions such as TBI (Traumatic Brain Injury), ABI (Acquired Brain Injury), stroke, Parkinson’s disease (PD), multiple sclerosis (MS), and more

I have had a great experience working with the longevity health and fitness. One of my clients had a below knee amputation as a result of a road traffic accident and really struggled with his fitness level when only completing physiotherapy sessions once a week. This was preventing him from progressing to a more active prosthetic limbs which would allow him to run again and this was one of his main goals. The personal trainer from Longevity worked really closely with the physiotherapist to increase his exercise tolerance, endurance and overall fitness by providing 2 additional sessions per week. My client has now progressed onto his high impact limb and is starting to run again, which he would not have been able to progress to without this additional support.

Key features for neurological conditions

Our areas of focus

For clients with neurological conditions, we focus on exercises that address both physical and cognitive function, improving overall wellbeing. Key features include:

  • Dual tasking: Exercises that challenge both cognitive and motor skills simultaneously, helping to improve coordination and brain function.
  • Balance/proprioception: Exercises to enhance balance and spatial awareness, reducing the risk of falls.
  • Cognitive function: Incorporating mental challenges alongside physical exercise to support mental clarity.
  • Postural stability: Strengthening exercises for core muscles to improve posture and prevent injury.
  • Coordination: Targeted activities to improve motor control and movement patterns.

Our process

Our process is simple and structured for the best outcomes:

  1. Consultation or joint session with a physio: We assess the client’s medical history, rehabilitation needs, and collaborate with the physio to create an integrated plan.
  2. Personalised exercise plan: We create a tailored exercise programme that aligns with treatment goals.
  3. Regular sessions: We deliver one-on-one sessions with clinical exercise physiologists or personal trainers.
  4. Ongoing support: Clients receive home exercise plans and regular follow-ups to track progress.

By integrating clinical exercise into the rehabilitation process, we ensure clients receive the support they need to achieve lasting recovery, improve mobility, and gain independence.
