Clinical exercise support for case managers

We understand the challenges of navigating the next stage in your client’s rehabilitation journey. Our expert team collaborates closely with physiotherapists and other healthcare professionals to create clinical exercise programmes that address your client’s unique needs and goals.

Focusing on achieving independence

Whether supporting home visits, transitioning clients into the community or gym, or introducing pool-based exercise, we tailor each plan to fit seamlessly with their ongoing rehabilitation.

Our approach focuses on exercise adherence, ongoing rehabilitation, and the development of independence.

How we support case managers and their clients

We work as part of a multidisciplinary team to ensure our clinical exercise plans are aligned with existing treatment plans and contribute directly to clients’ recovery goals. Our services cater to a wide range of conditions, including:

  • Post-injury rehabilitation
  • Chronic condition management
  • Long-term strategies for functional independence
  • Neurological conditions such as TBI (Traumatic Brain Injury), ABI (Acquired Brain Injury), stroke, Parkinson’s disease (PD), multiple sclerosis (MS), and more

“I have had a great experience working with the Longevity Health and Fitness. One of my clients had a below knee amputation as a result of a road traffic accident and really struggled with his fitness level when only completing physiotherapy sessions once a week. This was preventing him from progressing to a more active prosthetic limbs which would allow him to run again and this was one of his main goals. The personal trainer from Longevity worked really closely with the physiotherapist to increase his exercise tolerance, endurance and overall fitness by providing 2 additional sessions per week. My client has now progressed onto his high impact limb and is starting to run again, which he would not have been able to progress to without this additional support.”

Flexible exercise options

Our clinical exercise physiologists or clinical personal trainers deliver each session. Many clients find that working with a personal trainer provides them with the motivation and support they need, making the rehabilitation process both effective and enjoyable. We offer a variety of exercise options to ensure the most effective support for each client:

Home-based exercise programmes

Tailored to individual needs and delivered in the comfort of your client’s home.

Gym/community support

Guiding clients in local gyms or community settings to help them transition back into everyday life.

Pool-based exercise

Offering aquatic therapy where appropriate to reduce impact and enhance mobility.

Outdoor sessions

Exercising in open spaces or parks to encourage engagement in a natural environment.

Key features for neurological conditions

Our areas of focus

For clients with neurological conditions, we focus on exercises that address both physical and cognitive function, improving overall wellbeing. Key features include:

Strength

Resistance exercises using weights, bands or bodyweight help to strengthen the muscles in your back and core, reducing the strain on your back.

Mobility

Exercise which incorporates more stretching and mobility helps to relieve tension and compression in the low back, relieves pain, improves range of motion, and enhances muscle coordination to support spinal health.

Balance

Dance, Tai Chi and Yoga are all effective forms of exercise to improve balance and so prevent falls risk onto potentially fragile bones. These exercise styles may be especially beneficial on days when fatigue is more intense.

Cognitive function

Adding in a dual or cognitive task can increase the difficulty of an exercise and make it more relatable to everyday tasks that our clients are completing. It is an effective way to decrease falls risk and improve balance and cognitive function.
